GYMFUSION collects the personal information you provide in this form, including identity and contact details, demographic information, service preferences, work-related availability information and usual training availability, for the following purposes:
- (a)to verify your identity and maintain accurate records;
- (b)to assess and respond to your enquiry;
- (c)to determine your eligibility for our services;
- (d)to contact you regarding next steps; and
- (e)to support the provision of safe and appropriate coaching should you proceed with our services.
This collection is undertaken in accordance with the Privacy Act 1988 (Cth), including the Australian Privacy Principles (APPs), and, where applicable, the Health Records Act 2001 (Vic).
Collection of Information and Consequences of Non-Disclosure
GYMFUSION collects personal and, where relevant, sensitive information (including health information) directly from you.
This information is necessary to enable us to assess your eligibility, respond to your enquiry, identify potentially suitable service options and availability, and support the safe and appropriate provision of services should you proceed.
Some questions are optional or provide a ‘Prefer not to say’ option. However, if required information is not provided, we may be unable to complete the EOI process, assess your eligibility, identify suitable service options or contact you regarding your enquiry.
Use and Disclosure of Information
GYMFUSION may use and disclose your personal information for the purposes outlined above and in accordance with applicable privacy laws. This may include disclosure to third-party service providers engaged to support our business operations, such as providers of booking systems, billing services, form processing platforms, communication tools, and IT or data storage services.
All such disclosures are limited to what is reasonably necessary for the administration and delivery of our services and are managed in accordance with applicable privacy and data protection obligations.
Some of these service providers may be located overseas. Where this occurs, GYMFUSION takes reasonable steps to ensure that overseas recipients handle personal information in a manner consistent with Australian privacy laws. Further details are available in our

Data Security and Handling of Health Information
GYMFUSION takes reasonable steps to protect personal and health information from misuse, interference, loss, unauthorised access, modification, or disclosure. Health information is handled in accordance with applicable legal requirements, including the Health Records Act 2001 (Vic), and is only used or disclosed for purposes directly related to your health, safety, and the provision of services, unless otherwise permitted or required by law.

GYMFUSION encourages individuals to keep their personal information accurate and up to date. Where available, you may update certain details directly, or notify us of changes such as updated contact information or relevant health or injury information.
